Description
We are seeking a Nurse Practitioner to join our growing clinical team with considerable experience in newborn and early childhood care. You will serve patients across a seven-county region in southeast Indiana, conducting in-home visits and telehealth appointments on a schedule that works for you.
Key Responsibilities:
Provide primary pediatric care for infants and young children from birth through 36 months through in-home clinical visits, while following a novel care plan. Telehealth visits as needed.
Assessment and support of caregiver wellbeing through coaching and education.
Collaborate with BLOOM team, DCS, CASA, local health departments and other community resources.
Communicate with your patients and document encounters using our modern, AI-assisted platforms.
Participate in care coordination, trainings, team meetings, and call rotation.
