Attendee Directory Consent

ATTENDEE DIRECTORY CONSENT: I hereby give my consent for Synova to use and publish my personally identifiable information (“PII”) (described below) in the Attendee Directory which will be provided to all attendees of the Forum. I understand that the purpose of this directory is to provide attendees with a means to connect and network during and after the Forum.

By clicking to confirm consent, I acknowledge and agree to the following:

• PII: The PII that may be included in the public directory includes, but is not limited to, the following:
  • Full Name and photo
  • Job Title, Unit worked in, hospital and leadership information
  • Phone Number
  • Email Address
  • Organization/Company Name
  • City/State of Residence
• Directory Accessibility: The public directory may be accessed by other participants of the Forum, including Forum attendees, sponsors, exhibitors, and speakers. It may also be made available on the Forum’s official website or other Forum-related platforms.
• Privacy Considerations: While Synova will take reasonable measures to protect the privacy and security of the information included in the public directory, I understand that once the information is published, it may be accessed and used by individuals or entities outside the control of Synova. Synova shall not be held liable for any unauthorized use or disclosure of my PII by third parties.
• Withdrawal of Consent: I have the right to withdraw my consent for the inclusion of my PII in the public directory at any time by providing written notice to Synova. However, any information that has already been printed, published, or shared prior to the withdrawal of consent may remain in the public domain.
• Data Retention: Synova will retain my PII included in the public directory for a period of five (5) years following the conclusion of the Forum and/or my participation in any Forum with Synova, whichever is longer. After this period, the information will be securely deleted or anonymized.
• I have read and understood the terms of this consent and agree to its provisions. I willingly provide my consent for Synova to use my personally identifiable information as described above.
